These disorders are widespread and costly, and they exact a high toll on our Nation and around the world. The World Health Organization (WHO) determined the “burden of disability” associated with one major mental illness, unipolar depression, ranked fourth among all leading causes of disability worldwide. By the year 2020, the disease burden from depression will rank number two, surpassed only by heart disease (Murray & Lopez, 1996). The disability and disease burden of various medical conditions were estimated by “disability-adjusted life years,” or DALYs. This global burden has been underrecognized in economic cost and the impact on social structure.
